Please email us to reschedule your appointment. No refunds are provided for no-shows or cancellations but we are happy to reschedule.
After your appointment is confirmed, a confirmation email will be sent to you. In the email, a HIPAA secured link will open up an intake form for you to upload any medical records to review during your appointment. You may refer back to the same link in the confirmation email to upload additional information in the future.

Log into your MyChart account
On desktop, select “Share My Record” under “Health” on the top menu.
On mobile app, select “Share My Record” on the home screen.
Select “Give one-time access with Share Everywhere”
Give the share code to your provider and ask them to visit www.shareeverywhere.com.
Note: codes expire with 5 minutes of being generated. Once the provider logs into Share Everywhere, access expires after 20 minutes.

At the discretion of the provider, short term narcotic Rx may be prescribed for certain circumstances. All controlled substances requests would draw a query through the California drug monitoring program (PDMP). For example, post operative patients who are unable to reach their regular providers would be a reasonable scenario for a limited prescription. We do not provide schedule II/III refills for chronic pain patients, please reference your pain contract between you and your original prescriber.
Refills decisions are ultimately made at the discretion of the provider. A booked consultation does not guarantee refills.
No, we do not take insurance. Our fee-for-service model on request can provide you a super bill that will include relevant information such as procedure and diagnosis codes. If applicable, you may submit this information to your insurance for out-of-network claims. Our services are considered a qualified medical expense for HSA claims.
