We can access your health records through MyCHART or you may send your medical records securely through email. Additional visits may be scheduled as need.

Following exclusions apply

• Worker's comp evaluation/paperwork

• Controlled substance prescription for chronic pain management

• Immunization(s)

• Diagnostic tests
